Not every headache needs a neurologist β but some do. Miami-Dade patients: here are the five warning signs that mean your headaches deserve specialist evaluation at Viva Medical Center.
Headaches are the third most common pain complaint worldwide, affecting nearly 50% of the global adult population according to the World Health Organization. Most headaches are benign β tension-type or mild migraines that respond to OTC medications and resolve on their own. But some headaches are signals your nervous system is sending that demand serious attention. As a primary care and neurology team at Viva Medical Center in Doral, FL, here are the five warning signs we act on immediately.
Sign 1: The "Thunderclap" Headache β Worst of Your Life, Instant Onset
If you've ever experienced a headache that reached peak intensity within seconds β often described as "the worst headache of my life" or feeling like a bat hit the back of your head β this is a medical emergency. The thunderclap headache is the classic presentation of a subarachnoid hemorrhage (bleeding in the space around the brain). Call 911. Go to the ER. Do not wait to see if it resolves.
Even if it turns out to be a benign primary thunderclap headache (they exist), the diagnosis requires urgent imaging. Don't take the chance.
Sign 2: Headaches with Neurological Symptoms
Headaches accompanied by any of the following warrant same-day or urgent neurological evaluation:
- Sudden vision changes β double vision, loss of peripheral vision, visual aura lasting more than 60 minutes
- Weakness or numbness on one side of the body or face
- Slurred speech or difficulty finding words
- Loss of balance or coordination
- Confusion or altered consciousness
These symptoms can indicate a TIA (transient ischemic attack), stroke, or complex migraine with aura. The window for treatment of stroke is narrow. If symptoms are occurring alongside a headache, don't wait.
Sign 3: New Headaches After Age 50
The demographics of headache matter. Tension headaches and migraines typically begin in young adulthood. A new daily or recurring headache pattern in someone over 50 has a different differential diagnosis that includes:
- Giant Cell Arteritis (Temporal Arteritis) β an inflammatory condition that can cause blindness if untreated; responds rapidly to steroids
- Secondary headaches from hypertension, medication overuse, sleep apnea, or intracranial pathology
- Brain tumors β rare, but a new progressive headache pattern in this age group warrants imaging
Sign 4: Headaches Waking You from Sleep or Worse in the Morning
Migraines and tension headaches don't typically wake people from sleep. Headaches that are consistently worse in the morning or that disrupt sleep suggest elevated intracranial pressure β which can be caused by conditions ranging from idiopathic intracranial hypertension (pseudotumor cerebri) to mass lesions to sleep apnea-related CO2 retention. Your neurologist will likely order an MRI to rule out structural causes.
Sign 5: Progressively Worsening Headache Pattern
Any headache pattern that has been steadily worsening over weeks or months β in frequency, duration, or severity β despite appropriate treatment, needs imaging and specialist evaluation. This is especially true if OTC medications are no longer working or if you're using pain medications more than 10β15 days per month (which can cause "medication overuse headache," a treatable but often missed condition).
