5 Signs You Should See a Neurologist for Your Headaches

Sign 1: The "Thunderclap" Headache — Worst of Your Life, Instant Onset

If you've ever experienced a headache that reached peak intensity within seconds — often described as "the worst headache of my life" or feeling like a bat hit the back of your head — this is a medical emergency. The thunderclap headache is the classic presentation of a subarachnoid hemorrhage (bleeding in the space around the brain). Call 911. Go to the ER. Do not wait to see if it resolves.

Sign 2: Headaches with Neurological Symptoms

The demographics of headache matter. Tension headaches and migraines typically begin in young adulthood. A new daily or recurring headache pattern in someone over 50 has a different differential diagnosis that includes:

• Giant Cell Arteritis (Temporal Arteritis) — an inflammatory condition that can cause blindness if untreated; responds rapidly to steroids
• Secondary headaches from hypertension, medication overuse, sleep apnea, or intracranial pathology
• Brain tumors — rare, but a new progressive headache pattern in this age group warrants imaging

Sign 4: Headaches Waking You from Sleep or Worse in the Morning

Migraines and tension headaches don't typically wake people from sleep. Headaches that are consistently worse in the morning or that disrupt sleep suggest elevated intracranial pressure — which can be caused by conditions ranging from idiopathic intracranial hypertension (pseudotumor cerebri) to mass lesions to sleep apnea-related CO2 retention. Your neurologist will likely order an MRI to rule out structural causes.

Sign 5: Progressively Worsening Headache Pattern

Any headache pattern that has been steadily worsening over weeks or months — in frequency, duration, or severity — despite appropriate treatment, needs imaging and specialist evaluation.
